“Now you can buy that medication at a pharmacy without going to a hospital!”
A major shift is currently taking place in the medical world.
A growing number of medications that traditionally required a doctor’s prescription are moving toward over-the-counter availability.
Among the most talked-about are the ED treatment Cialis (generic name: tadalafil) and the sleep-disorder medication Rozerem (generic name: ramelteon).
“For people who have hesitated to seek treatment out of embarrassment, this is an opportunity to dramatically improve their quality of life (QOL). When used properly, there is no doubt that these medications can be extremely effective.”
That is according to Dr. Tetsuya Kubota, director of Kubota Clinic, who specializes in ED treatment and male menopause care.
The effectiveness of these medications is well established. Precisely because of that, however, medical professionals including Dr. Kubota are concerned that making them available over the counter could lead people to treat them like casual supplements while overlooking the risks.
<h2>The overwhelming effectiveness of the &#8220;Weekend Pill&#8221; and the one combination that can be deadly</h2>
Because its effects can last up to 36 hours, Cialis is often called the “weekend pill.” It is an extremely useful treatment for ED. It is not known for causing severe side effects and is widely trusted in clinical practice, having helped many men struggling with the condition.
However, Dr. Kubota warns:
“A medication being relatively safe does not mean it is a drug that anyone can use freely without concern.”
The greatest risk associated with Cialis is taking it together with nitrate medications, which are commonly used to treat conditions such as angina.
“If a patient taking medications such as nitroglycerin or isosorbide nitrate also takes Cialis, there is a risk of a sudden and dangerous drop in blood pressure. The problem is that many patients do not accurately understand the medications they are already taking. Quite a few people either do not bring their medication records or do not check them.
If you do not understand what medications you are taking, you cannot recognize dangerous drug interactions. Going forward, these drugs may be sold based largely on self-reporting. There is considerable concern among healthcare professionals about whether safety can truly be confirmed that way.”

In many cases, erectile dysfunction itself can be a warning sign of a serious underlying illness.
“Sometimes the cause of ED turns out to be vascular conditions such as diabetes, arteriosclerosis, or hypertension. At a medical institution, we do not simply prescribe medication—we also check for underlying diseases. If people can simply buy the drug at a pharmacy and that is the end of it, there is a risk that these SOS signals will go unnoticed.”

<h2>The hidden catch behind promoting natural sleep</h2>
Another medication drawing attention is Rozerem, which is scheduled to become available over the counter starting in July. In medical settings, it is known as a highly safe medication and is often praised for its ability to promote natural sleep.
“Rozerem’s active ingredient, ramelteon, works in a similar way to the sleep hormone melatonin, helping regulate the body’s internal clock and naturally induce sleepiness. Unlike conventional sleeping pills, which strongly suppress brain activity, it is less likely to cause dependence or tolerance.”
Although it is considered a safe medication, there is still an important line that users need to be aware of.
“Its use together with the antidepressant fluvoxamine (Depromel, Luvox) is contraindicated. When taken together, blood levels of ramelteon can rise significantly, increasing the risk of side effects such as severe drowsiness, excessive sedation, dizziness, and unsteadiness. Pharmacists must carefully check for this before selling the medication.
There is also a risk that drowsiness or dizziness may persist into the following morning. When combined with alcohol, unexpected side effects such as increased sleepiness, reduced attention, impaired judgment, and a greater risk of falls may occur. People who drive or perform hazardous work should be particularly cautious.”
Persistent insomnia after taking the medication may also be a warning sign.
“Conditions such as depression, sleep apnea syndrome, and menopause-related disorders are often hidden behind insomnia. People may think, ‘I can’t sleep, so I’ll take a pill,’ but moving beyond that point requires awareness of the potential risks.”
There is no doubt that both Cialis and Rozerem are excellent medications. However, the greatest challenge in making these drugs available over the counter and ensuring they are distributed safely lies in the screening process at pharmacies.
“Many patients do not know the official names of the other medications they are taking. There is concern among healthcare professionals about whether it is really possible to confirm safety 100 percent based solely on self-reporting,” says Dr. Kubota.
In the case of Cialis, the nature of the product as an ED treatment can further increase the risks because of human psychology.
“To obtain it through a hospital, patients must undergo a doctor’s examination. Some people will inevitably think, ‘I don’t want to answer a lot of detailed questions, so I’ll just hide my pre-existing conditions and buy it at a pharmacy instead.’ In medical institutions, risks are carefully assessed while referring to the patient’s medical records. Pharmacies, however, are busy environments, and pharmacists and staff are often pressed for time. The key question is how much time can realistically be devoted to risk assessment.”
One possible way to reduce risks would be to use Japan’s My Number Card system to verify medication histories and medical records. However, the necessary infrastructure is not yet fully in place, and challenges remain in ensuring safety.
The government’s push toward self-medication has the potential to help many people struggling with these conditions. Precisely because of that potential, proper risk management will be essential.
